This is a 12-month project commencing in December 2010.

Purpose:

This study will systematically review the research and other key documents for best practice palliative care clinical tools and outcome measures. It will provide advice and consensus recommendations to the Palliative Care Clinical Network for state-wide implementation of the clinical tools and outcome measures to improve clinical expertise and encourage best use of resources in the Victorian palliative care sector.

Method:

A systematic review of the literature will be conducted, which will focus specifically on care planning and care pathways (including “Liverpool Care Pathway”) and identification of relevant tools that support this process.
