The Liverpool End of Life Care Pathway

The Liverpool End of Life Care Pathway (LCP)  provides a strategic framework to guide multi-disciplinary care in the last days and hours of life, enabling the provision of optimal quality of care. The LCP was designed to address 18 key goals including symptom control, comfort measures, prescribing practices (including anticipating analgesia and sedation), discontinuing inappropriate interventions and care of the family [6, 8-10].

The LCP was developed to replace all existing paperwork and becomes a key legal source of documentary evidence of decision-making and care provisioning. It is designed to facilitate internal and external audits and benchmarking, and outcome measurement [8, 9].

Originally developed for inpatients and based on the hospice model of care, the LCP has been since been implemented in a range of settings including residential aged care and oncology.
