Other Tools for Use in the Palliative Care Sector
A number of projects and resources have been developed to aid in determining care provisions and to provide support for quality improvements in the palliative setting. These resources may be used in conjunction with an end of life care pathway, further developing the framework of a continuous collaborative approach to providing care during all stages of palliation.
The review of Clinical Assessment Tools in the Palliative Care Project is currently underway and is expected to be completed in December 2011. The project will include a review of the literature concerning available palliative care clinical assessment tools with the aim of aiding and coordinating palliative care provisions across all health care settings in Victoria.
